(Appointments vary on provider availability, most appointments scheduled Monday - Thursday evenings)
New Patients MUST complete prequalification paperwork prior to scheduling any appointments
Prequalification Enrollment Hours
Mondays 1:00 pm - 5:00 pm
2nd & 4th Tuesdays 1:00 pm - 5:00 pm
Prequalification enrollment requirements include:
Photo ID
Proof of Household Income (2 consecutive pay check stubs for all members of household or most recent tax return)
All paperwork is renewed at the change of each calendar year for all patients.
